Summary

The Fargo VA Health Care System is a Joint Commission accredited medical/surgical hospital with 36 acute care beds - a 38 bed Community Living Center - Primary Care and Specialty Medicine Clinics - 10 Community Based Outpatient Clinics - and one Community Resource Referral Center. We oversee the provision of health care to more than 34 -000 Veterans living in North Dakota - 17 counties in northwest Minnesota - and one northeast South Dakota county.

Duties

• The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers
• This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest
• As an employee operating in a Whole Health System of care - you will operate in a model with three core elements - seeking to create a personalized health plan for each Veteran
• This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community
• This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being
• The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
• The NP provides advanced clinical practice - consultation - and management.Duties include: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Providers may be required to travel to the CBOC's or provide gap coverage as determined by the supervisor
• Provider will be required to be a designated women's provider and complete the mandatory VHA training
