Growing together.As the Nurse Practitioner, you will be responsible for providing an advanced level of nursing practice to patients and families and a clinical resource to staff and the interdisciplinary team.
Primary Responsibilities:
- Delivers specialized primary care and consultation, including prescriptive authority and on-call duties, in collaboration with a supervising physician
- Coordinates and manages care for patients with complex conditions, including conducting comprehensive assessments and directing interdisciplinary team meetings
- Performs face-to-face patient visits and ensures proper documentation using company-defined processes and forms
- Provides clinical education internally and externally, and serves as a resource for staff while participating in team meetings and quality improvement initiatives
- Supports performance improvement and fulfills additional duties as assigned, contributing to overall clinical excellence

Required Qualifications:
- Current unrestricted license in the state of residence
- Certified as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice
- Current certification by a national body such as ANCC or AANP
- Possession of NPI Number, DEA with prescribing privileges, PECOS enrollment, CAQH enrollment, Medicare number, and referable Medicaid number
- 1+ years of NP experience in chronic disease management, family care, or primary care
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation
Compensation for this specialty generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
