Description

Family Nurse Practitioner

Position Description:  A dynamic, team-oriented Family Nurse Practitioner (FNP) who is skilled in the provision of primary / urgent care for the managed Medicaid population. The ideal candidate will function independently in a collaborative environment. This candidate will be responsible for conducting patient visits related to acute, chronic care and preventive care management.  They will be responsible for accurately coding and documenting all visits in partnership with the managed Medicaid plans quality initiatives.  Of utmost importance, this role is for an individual comfortable in a startup environment where an “all hands-on deck” mentality is key in fostering a positive team feel in a time of great growth.

Some Essential Job Functions Include:

  • Confidence and comfort in assessing the patient initial evaluation, history, and physical examination of patients whether they are primary care or urgent care patients
  • Make decisions regarding data gathering and appropriate management and treatment for the initial evaluation of a problem or the follow-up of a previously diagnosed condition
  • Provides patient care management within advance practice parameters; manage acute care and chronic care activities
  • Assist team members with inbound clinical triage
  • Identify, develop, and implement clinical protocols for patient treatment
  • Assist with patient education activities
  • Conduct presentations and community sessions pertaining to disease specific process and wellness
  • Monitor the quality of care delivered to our members
  • Document patient contacts appropriately; complete chart reviews on a regular basis and in a timely manner
  • Provide and receive feedback on quality of care being delivered regulatory standards are met

Requirements

Qualifications:

  • Graduate of an accredited program for Family Nurse Practitioners (FNP)
  • Continued education and training in the diagnosis and management of common medical conditions and chronic illnesses
  • Current licensure as a Nurse Practitioner in state of [Pennsylvania and/or NJ]
  • BLS, ACLS and PALS Certification.
  • Current Certification by the American Academy of Nurse Practitioners or American Nurse Credentialing Center
  • Meets all requirements to obtain associate medical staff membership and required privileges of service.
  • Eligible for a DEA and NPI #
  • Experience with EMR records
  • At least 1-2 years of experience in primary care, preferably in a family practice setting that treats pediatric patients
  • Experience in an urgent care environment a plus
